A question for my blogging friends

Last year my listening center was strictly that--listening. They listened to the book, and if time allowed they listed again. This year I'd like for there to be a sheet they need to fill out. It would have to be pretty simple at the beginning for the year, but could advance from there. Do any of my blog friends havea sheet like this that they could share?
